I just use those, nothing else. But I have also tried other combinations but
nothing I try makes any difference whatsoever, filename stay exactly the
same.
I tried your example %h%p/%[xxx::_::]%n%q.%t now with same result.

I use Action: Get separate files but tried other actions too. Nothing I try
makes any difference.
