1) The disk is full
2) An open file has filled the disk but its directory entry has been deleted
3) The output directory is either read only or you do not have permission to
write there.
4) your outputting somewhere unintended

A reboot and dir should show 1,2
Either Attrib or explorer w/ Attributes should show #3
