You don't make a form, the form is already in the html code.

You have to make the server code that executes when the form is clicked and
returns a new html page as its result.

You don't HAVE THAT CODE. You don't know what the button does, nor what every
combination of parameters produces.

Get busy coding. After a few man-months you should have something similar to
what the original site already does.
